ORTHOPEDIC & SPINE SURGERY

The department or Trauma, Orthopaedics & spine surgery was founded in 1996 and is one of the busiest units in the country. We take pride in the services we provide and our skilled and dedicated staff. It is one of the only units in the country where dedicated subspecialty services are provided to our patients under the care of fully trained and qualified consultants in their relevant fields.

The recently renovated fully air-conditioned department has the capacity to accommodate 110 in patients, with separate bays for Spine, Arthroplasty, Oncology, Paediatric and infected cases. We provide specialist care for simple and complex trauma including pelvis and acetabulum fractures, non-union and deformity correction, primary and revision arthroplasty, spine pathology, Paediatrics, Oncology and sports injuries. We are a tertiary referral unit for many of these subspecialties and also provide care to patients from neighbouring Afghanistan.

Our academic unit provides educational facilities for undergraduate medical students from Khyber girls medical college and is one of the most reputed unit for post graduate training in Trauma & Orthopaedics in KPK. Numerous post graduate trainees from our unit have progressed on to consultant posts after completion of their training and passing their FCPS Orth exam. We are also one of the only units in the country to provide post FCPS advanced fellowship training in spine, arthroplasty, oncology and paediatrics and we have fellows from all over the country for short- and long-term training.

Alongside clinical activities, we are a progressive and academic unit and organise regular courses for trainees in trauma, arthroplasty, spine, oncology, paediatrics and exam preparation on a fixed annual calendar. Our consultants are regularly invited as faculty both nationally and internationally in various meetings, courses and seminars to share their knowledge, skills and experience. We have an extended database of all our patients in different subspecialties where we use this data for extensive research and publications.

Our unit gets extensive support from diagnostic facilities within the hospital like pathology labs, radiology including plain radiography, Ultrasound scan, CT scan, MRI scan and well-equipped physiotherapy department with dedicated physiotherapists for our unit. We work in close cohesion with various other units within the hospital and outside in other hospitals to provide total care for our patients.

In summary, we aim to deliver high quality, evidence-based care to our patients which is acceptable to them in their environment with a constant drive to improve our services. At the same time, we take pride in our academic services at undergraduate, postgraduate and post FCPS level and continue to expand on our academic involvement.

1.Trauma & deformity correction.

 

We are one of the major trauma units of the province and provide an extensive service dealing with all types of simplex and complex trauma including pelvis and acetabulum fractures and spine trauma. We also deal with significant number of patients presenting with simple and complex non-unions along with deformities requiring complex surgical procedures. We operate on approximately 3000 cases per annum of patients with different fractures, deformities, simple and infected non-unions.

 

We use the most advanced surgical techniques and implants available for dealing with these fractures including open reduction internal fixations, percutaneous fixations including Kirschner wires, minimally invasive percutaneous osteosynthesis, Interlocking nails and intramedullary nails, external fixators including simple fixators and Ilizarov fixators. We also deal with a significant number of patients with hip fractures and use dynamic hip screws, dynamic condylar screws, proximal femoral nails and where needed locking plates for fixation. For patients with intracapsular hip fractures we perform hemiarthroplasty and total hip arthroplasty using various implants of high repute.

 

All our patients with fragility fractures are provided with antiresorptive therapy and receive VTE prophylaxis according to a departmental approved policy. Post operatively, our patients have a dedicated physiotherapist on the ward for mobilisation and they are followed up in outpatient’s clinic. We take pride in the quality of patient care that we provide along with our results which are according to any internationally acceptable standards.

 

2.Spine Surgery

Spine Unit of HMC was established in 2012, as the first spine unit in any govt hospital in the whole country. Being the only Spine unit, we receive patients from all over the country and neighbouring Afghanistan. Here we perform all types of Spine surgeries including:

 

Endoscopic Spine surgery

Minimally invasive Spine surgery

Deformity Correction

Degenerative Spine Diseases.

Spine Tumours.

Spine fractures.

Spine Infections.

Metabolic diseases of the Spine.

 

Apart from patient care, we also run a structured Spine Fellowship program for fellows from all over the country and abroad. Along with fellowships, we also provide training to FCPS orthopaedic residents and Undergraduate medical students.

 

3.Arthroplasty

 

The arthroplasty team is comprised of fully trained arthroplasty surgeons providing both primary and revision arthroplasty services for hip and knee joint. We provide arthroplasty services for degenerative, inflammatory and traumatic conditions effecting these joints utilising the latest and most advanced techniques and implants. For complex revision arthroplasty cases, we also perform reconstruction with megaprosthesis. All the cases performed in our unit are routinely entered into the Pakistan national joint registry for research and evaluation purposes.

 

All the post-operative patients are managed by a dedicated physiotherapist and post-operative care is provided according to international standards tailored to our needs. These patients are followed up in our outpatient clinic for life time of the prosthesis/patient.

 

 

 

4.Oncology

 

Our oncology service is the first in KPK to provide dedicated services for patients suffering with bone and soft tissue tumours under the care of fellowship trained oncology surgeon. We deal with both primary and secondary bone and soft tissue tumours and perform major procedures including limb salvage surgery utilising biological reconstruction including autograft (Both vascularised & non-vascularised), recycled bone and megaprosthesis according to internationally agreed protocols and latest evidence. We work as referral centre for all the hospital’s in KPK including major oncology hospitals. We work in close liaison with our plastic surgery colleagues for microvascular services and medical and radiation oncologists in a multidisciplinary setting. On average we deal with approximately 250 surgical cases every year which also includes major pelvic tumour surgeries. Our unit is the first unit in the country to provide post FCPS training for further advanced training in Orthopaedic Oncology and limb reconstruction.

 

 

5.Paediatric orthopaedics

 

As with other subspecialties, the division of Paediatric Orthopaedics was established in 2016. Since then it has grown rapidly to an extent that it has become a centre of referral to all kinds of Paediatric Orthopaedic conditions for the whole province and beyond. It is recognised for subspecialty training, every year two fellows undergo advanced training in this division, once they are fully trained, they can start practicing paediatric orthopaedic services in their own location. Our postgraduate FCPS trainees also undergo a 3 months rotation in this division with an intense theoretical and practical experiences. In the department most of the latest and advanced management of children with trauma and its related conditions, congenital, developmental, metabolic conditions along with paediatric tumours (with orthopaedic oncology colleagues) and deformity correction (in collaboration with our Ilizarov fixation colleagues). Surgical procedures are performed two days a week, yearly around 300 surgical procedures are done in the department.

 

Our department is also well known for its routine and up to date academics. Every patient seen in the outdoor as well as inpatients are discussed thoroughly before the final diagnosis and management plan is made. There are numerous articles published from our unit, there is also a lot of on-going research. Also, numerous papers have been presented by consultants and trainees in both national and international conferences. Our faculty is also frequently invited for lectures at national and international levels.

 

 

 

6.Sports injuries

 

Department of Orthopaedics and Spine ha specialized Sports Surgery unit with dedicated OPD and theatre.

It has one Associate Professor and two Specialist Registrars. All sorts of sports surgical procedures are performed in this center of excellence. This unit has special interest in Arthroscopic knee multi-ligament reconstructions, meniscal repair, Osteotomies around the knee and arthroscopic Shoulder and Elbow surgeries, AC joint reconstruction and Shoulder Replacement.

The department of orthopaedics and spine surgery has always been at the forefront of providing training to young and budding orthopaedic surgeons. All our faculty are actively involved in teaching and training both at national and international levels. To further enhance the training opportunities for young doctors and subspecialisation, this unit offers post FCPS approved fellowships from Pakistan orthopaedic association in the following disciplines:

  1. Spine surgery.
  2. Arthroplasty and adult reconstruction.
  3. Oncology.
  4. Paediatrics.

After completion of 12 months of rigorous training, these consultants then take up their posts in their own districts and provide these advanced services to the population at their door step.
